panic at re-evaluating plan step

When running an agent with memory enabled and github actions, LocalAGI crashes repeatable after some time with a panic:

Start at github.com/mudler/cogito's plan.go:230 and follow ExecutePlan's re-evaluation path after plan.go:289 reports that all attempts failed; then inspect core/agent/agent.go:1150. Reproduce with memory and GitHub Actions enabled, and confirm the agent no longer panics while re-evaluating a failed plan.
