Show host name in Agent Host Terminal tab titles

Description
When multiple Agent Host terminals are open for different remote hosts or tunnels, every tab is titled `Agent Host Terminal`. This makes it impossible to tell which terminal belongs to which host.
The tab title should include the remote host or tunnel name, for example:
- `Agent Host Terminal - agents-1`
- `Agent Host Terminal - agents-2`
This would make multiple Agent Host terminal sessions distinguishable at a glance.
