Discover chats created by other Codex clients without reconnecting the remote tunnel

## Summary

Agent Window should discover chats created by other Codex clients, such as the
Codex VS Code extension, while the remote tunnel remains connected.

Currently, the chat list appears to depend on the initial `thread/list` result
and connection-local `thread/started` notifications. Chats created through a
different client are not discovered until the tunnel is disconnected and
reconnected.

## Problem

When multiple clients share the same remote Codex session storage:

- Agent Window is connected through a remote tunnel.
- A new chat is created using the Codex VS Code extension.
- The chat is persisted successfully.
- Agent Window does not display it, even after refreshing its interface.
- Disconnecting and reconnecting the tunnel causes the chat to appear.

Example: a chat named `Diagnosticar chats no visibles` only appeared after
reconnecting the tunnel.

## Expected behavior

Agent Window should discover persisted chats created by any compatible Codex
client without requiring a tunnel reconnection.

## Suggested implementation

Reconcile the recent conversation list using `thread/list` through one or more
of these mechanisms:

1. Poll `thread/list` at a reasonable interval while the chat list is visible.
2. Refetch when Agent Window or the chat list gains focus.
3. Ensure the refresh action invalidates the cached recent-conversations query
and performs a real `thread/list` request.
4. Optionally watch the shared session database/storage for changes.

This should complement connection-local `thread/started` notifications rather
than require every external Codex client to broadcast events to Agent Window.

## Acceptance criteria

- Connect Agent Window to a remote workspace through a tunnel.
- Create a chat from the Codex VS Code extension.
- The new chat appears in Agent Window without reconnecting the tunnel.
- The update happens automatically or after using the refresh action.
- No duplicate chats are introduced by reconciliation.
