Steps to Reproduce:

1. Start a new session, and have it make a change
2. 'Keep' those changes and notice the sesssion summary doesn't update

From this doc https://code.visualstudio.com/learn/foundations/agent-sessions-and-where-agents-run I think this is 'wrong'. But... this recently -- this change https://code.visualstudio.com/docs/agents/run/sessions/manage-sessions seems to indicate this is an intentional change? But it should then be all teh changes for the whole session and not just the most recent ask? And then how are we supposed to know a session has changes that we need to review... 🤔
