Too many ways to pick mode for Claude harness

Does this issue occur when all extensions are disabled?: Yes/No

- VS Code Version: 1.134.0
- OS Version: Windows 11

When using the Claude harness, picking the mode is very confusing because there are two locations to pick agent vs plan mode etc.

For example: here I have picked "Agent" and "Plan Mode":

But I can also pick "Plan" and "Plan Mode":

The situation is confusing and it is hard to understand what is what.
