Keep/Undo covers up the new text

Type: Bug

When reviewing agent-created edits to a file, the Keep and Undo buttons are located directly over the top of the new contents, making it impossible to properly review them. Just like many other hovers throughout VScode (suggestions and completions at minimum) who consistently get it wrong, there should *never* be a situation in which a hover is placed over any part of the context for which the hover exists.
