"Add feedback" and `/act-on-feedback` go overly narrow

I found myself wanting to ask questions by selecting code in Changes and asking model to explain.

However, `Add feedback` in UI and `/act-on-feedback` slash command later leave me a bit in confused/unsatisfied state because I actually want to ask a question and not provide feedback.

I think we should try having more general wording "Comment..." or something
