GitHub Copilot Chat intermittently displays the Claude Code chat interface

- Copilot Chat Extension Version: 0.48.1
- VS Code Version:1.116.0 (Universal)

- Feature (e.g. agent/edit/ask mode): chat

Steps to Reproduce:

1. Open Visual Studio Code.
2. Open GitHub Copilot Chat using the Copilot Chat icon or command.
3. Use Copilot Chat normally.
4. Close and reopen the chat panel, switch between files, or reopen the workspace.
5. Occasionally, the Claude Code chat interface appears instead of the GitHub Copilot Chat interface and history shows the codex items

## Expected behavior

Opening GitHub Copilot Chat should always display the GitHub Copilot Chat interface.

The Claude Code chat interface should only appear when I explicitly open Claude Code or select its chat view.

## Actual behavior

The Claude Code chat UI occasionally appears in the Copilot Chat area after I open GitHub Copilot Chat.

## Frequency

The issue occurs intermittently and is not consistently reproducible.
