Agents, /code-review command conflicts with Claude Agent SDK

Description

Does this issue occur when all extensions are disabled?: Yes

• VS Code Version: 1.127
• OS Version: windows

Steps to Reproduce:

1. launch Agents host session window (https://code.visualstudio.com/docs/agents/agents-window#_open-the-agents-window)
2. switch to claude agent sdk in local agent host dropdown
3. type /code-review (popup-will be correct built-in claude sdk command),

5. enter but then switches to "wrong" agents built-in skill instead (https://code.visualstudio.com/updates/v1_126#_agentic-code-feedback-with-agent-host-harnesses),

verified when running review it does not use the full claude code-review flow version
