[Bug]: no connection to server

### Description

Error message "The SQLToolsService service has crashed. Details: The service process was unexpectedly closed" upon startup of VS Code. Unable to connect to MSSQL.

Reproduce the failure by starting VS Code and attempting an MSSQL connection, then trace the SQLToolsService crash reported in the error message. Done means the service remains running after startup and an MSSQL connection succeeds on the stated current versions.
