Description
Repro is at the location of the function call is_write_opname_kind. Restarting the IntelliSense client doesn't fix it but switching documents does.
The region is disabled when it should be enabled (the define is set by the configuration provider, which I'm guessing is a requirement). It might also require a big enough file.

Research direction
Reproduce the issue at the function call is_write_opname_kind with a configuration provider and an initially opened, possibly large file. Compare inactive-region state before and after switching documents, and inspect how the provider's define is applied. Done means the region is enabled correctly on initial open without switching documents.
