Description

I don't have a solid repro yet, but I was hitting it previously and this seems to be the case from code review.

I'm not sure if anyone else is hitting this so it seems low priority.

Research direction

Start by tracing the Windows refactoring operation that creates a file when it exists in the database but not on disk, then compare the stored header-file name with the filesystem result. Reproduce the case if possible and verify that refactoring no longer creates an incorrectly all-caps header file.
