Description

Debug logging will show "LSP: $/cancelRequest (cpptools/findAllReferences" sometimes the UI will shows "Canceled", but sometimes it won't (not sure why that is behavior is sometimes different).

Research direction

Reproduce the issue by using Go to Symbol in Workspace or Go to Definition while Find All References or Rename is running, then observe changes to files.associations and the cpptools/findAllReferences cancellation log. Compare the debug log with the UI's Canceled state; done means these navigation actions no longer unexpectedly cancel the active reference or rename operation, with consistent UI behavior.
