Description
Repro is just to rename a folder that appears in an Add to include code action. Doing a Rescan Workspace also fixes it.
I hit this while investigating the repro for another issue.

Research direction
Reproduce the issue by renaming a folder shown in an Add to include code action, then inspect the extension entry point that supplies those suggestions and how workspace rescanning refreshes them. Done means renamed folders no longer leave old suggestions, without requiring Rescan Workspace; no file or test is named in the issue.
