Research direction
Start by tracing container app creation from the RESOURCES view, the FOCUSED RESOURCES view, and the "Azure Container Apps: Create Container App..." command in the command palette. Reproduce the issue using the listed steps and inspect how the focused view refreshes after creation. Done means a newly created container app appears automatically in the FOCUSED RESOURCES view for each creation entry point.
