[Bugbash] CoR workflow failed to switch to scaffold from plan

The workflow generated the .azure/project-plan.md but it didn't know that planning has completed. The main window view keep showing a "Generating your project plan…" and in the chat window, the agent stays in the plan subagent. It asked me if I want approve the plan in the chat window. After saying yes, the agent started generating the implementation on its own in the plan subagent.

The response claims that "azure-project-scaffold" hand-off tool isn't available.

After approving the plan it went on to implement it in plan subagent.

I am using latest VS Code Insider build.

Reproduce the CoR workflow using the reported project-plan output and inspect the transition from the plan subagent to the azure-project-scaffold hand-off. Verify that approval leaves the generating state, switches to the scaffold workflow, and prevents implementation from continuing in the plan subagent.
