Investigation needed into the start-extension failure in vscode telemetry data

From the recently vscode telemetry data, errors are reported in ~10% start-extension telemetry event. Though these failures should be mostly related to user's environment (i.e. whether typescript compile installed properly), but ~10% still feels a little too high. It's good to do some further investigation there (may need some additional log) to see whether we are able to get a clearer picture there.
