Unable to add or refresh Auth profile

I have installed VS Code again from Store, removed and installed the Power Platform extension. PAC is working in terminal, but when I try to add auth profile or refresh, I'm seeing errors:
New profile:
Error running command pacCLI.authPanel.newAuthProfile: command 'pacCLI.authPanel.newAuthProfile' not found. This is likely caused by the extension that contributes pacCLI.authPanel.newAuthProfile.

Refresh:
Error running command pacCLI.authPanel.refresh: command 'pacCLI.authPanel.refresh' not found. This is likely caused by the extension that contributes pacCLI.authPanel.refresh.

Clear:
Error running command pacCLI.authPanel.clearAuthProfile: command 'pacCLI.authPanel.clearAuthProfile' not found. This is likely caused by the extension that contributes pacCLI.authPanel.clearAuthProfile.

After installing the extension, I'm seeing notification:
Preparing pac CLI (v2.8.)...
Source: Power Platform Tools, and it stays forever. It isn't getting completed.

Start by reproducing the authentication profile commands in VS Code 1.124.2 with Power Platform Tools 2.0.145 on Windows, then inspect the Power Platform Tools output and the stuck “Preparing pac CLI” notification.
