"Description" hardcoded in English when converting images to another language

This is hardcoded: and it should not:

If I am passing a prompt, let ME control if I want a "Description" heading or not, and in what language it should be.

Suggestion: only append it if the prompt is None.

Locate the image-conversion code containing the shown markdown assembly and inspect how llm_prompt controls description generation. Verify that a custom prompt does not force an English heading, while the default behavior remains appropriate; confirm the resulting markdown for both prompt cases.
