Description
Issue triage was over-creating issues and bloating the backlog. Two root causes: the agent decomposed issues into sub-issues, and the workflow allowed create-issue (up to 5 needs-triage issues per run).
Scope:
- Remove decomposition from the issue-triage agent and the workflow create-issue safe-output.
- Fold the repository label taxonomy (type/area/priority) into the agent and expand dd-labels.allowed.
- Keep eval expectations and the compiled lock in sync.
