Description
Created work items do not always carry the required labels/tags.
Desired behavior
- Every generated work item receives its required labels per the work-item-creation skill.
- Missing labels are created or the item creation surfaces the gap.
Acceptance criteria
- Label application is enforced consistently at creation time.
