Start with UpgradeActionRegistrar.java at tryAddToGroup (line 100) and discoverAndRegisterAction (line 63), then inspect JavaUpgradeContextMenuAction in the com.microsoft.azure.toolkit.intellij.appmod.javaupgrade.action package. Reproduce the IntelliJ exception and trace why the action is registered twice; done means the duplicate-action error no longer occurs.
