Add “Add and Apply Migration” command support (EF Core 11)

### Is your feature request related to a problem? Please describe the problem.

Users commonly want to scaffold a migration and immediately apply it. EF Core 11 supports this with a single command; Aspire should offer a first-class experience for the same flow.

### Describe the solution you'd like

EF Core 11 introduced a unified “Add and Apply migration” workflow. `Aspire.Hosting.EntityFrameworkCore` migration management integration (introduced in https://github.com/dotnet/aspire/pull/13481) should provide an equivalent command/workflow.
