Description
Edit 6/9: This is a Command Palette bug. See https://github.com/microsoft/PowerToys/issues/39936 for updates.
There's a repro in the GitHub extension as well. It's not yet clear if the bug is an extension bug or a CmdPal bug.

Research direction
Start with the linked Command Palette issue #39936 for the current status and scope, then reproduce the missing toast through the GitHub extension when editing an item opened from a top-level item. Done means the responsible component is identified and toast messages appear on the edit page; the issue does not name a file or test.
