Description
The opt-out form can include external stylesheets, which we should support proxying in the matomo-proxy.php endpoint (along w/ images).
There is a simple workaround, however, so this isn't too high a priority.
Workaround:
1. put your external stylesheets/assets in your website that houses the tracker proxy
2. in matomo, configure the external stylesheets to use URLs w/ your website domain

Research direction
Start with the matomo-proxy.php endpoint and trace how it currently proxies images. Check how the opt-out form references external stylesheets and assets, then determine the corresponding proxy behavior. Done means external stylesheets and assets used by the opt-out form are proxied, while the documented workaround remains unnecessary.
