Description

Hi! I just encountered a problem when folding, if I have the following markdown
in a file:

> \# First
>
> \```
> 1
> 2
> 3
> 4
> 5
>
> 7
> \```
>
> first
>
> \* first
> \* second
> \* third
>
> \# Second
>
> second

Then when I close the fold it 'eats' the second heading. So it goes from

Let me know if you can reproduce it. I tried only loading this plugin but
the issue persists. I'm using nvim if that helps.

Research direction

Reproduce the folding problem in nvim v0.4.3 using the Markdown example in the issue, including the fenced block and the second heading. Inspect the plugin's folding behavior around these sections. Done means closing the fold no longer hides or consumes the “# Second” heading.
