Current behavior

The "transition" history tab appears to be missing a record of some transitions. Here, we can see that the ticket jumped from Todo to Complete, when there were transitions that occurred in between: from Todo -> New, likely then to In Progress, then eventually to Complete. I noticed this when this work item seemed to have been inadvertently marked Complete by someone and I needed to reopen it.

Here is another example. Because our work item workflow starts in the Todo state, there is no way that the In Review -> In Progress transition could have been the first transition to occur (it needed to go from Todo -> New -> In Review first).

Steps to reproduce

Not sure how to reproduce. Here is an example of what I would have expected the transition tab to look like for the second bad example above (this is an example of it working correctly):
