Codex session 偶发进入 5/5 重连失败并无法恢复

## 现象
在使用 Codex session 的过程中，偶尔会出现连接异常。当前 session 会一直显示“重连中”，直到重连次数达到 5/5，随后提示连接失败。

## 复现步骤
1. 正常使用一个 Codex session。
2. 在没有明确规律的情况下，session 进入重连状态。
3. 等待自动重连完成。
4. 重连尝试达到 5/5 后失败。

目前无法稳定复现，也没有发现明确的触发条件。

## 期望行为
当前 session 应自动恢复连接并继续工作，同时保留已有上下文。如果多次重连仍失败，希望提供可用的手动重连或恢复入口，并显示更明确的失败原因。

## 实际行为
发生后当前 session 基本无法继续使用，重连不会自行恢复；只有新建一个 session 才能正常工作，原 session 中的上下文和未完成工作因此受到影响。
