Disturb cards exile ability triggers when surveiled to graveyard

The likely explanation is that the PutIntoGraveFromAnywhereSourceAbility from the back face is somehow active, although it shouldn't be

Reproduce the issue with Lunarch Veteran or another disturb creature, using a surveil effect that puts it into the graveyard. Trace the tdfc rework around PutIntoGraveFromAnywhereSourceAbility on the back face; done means surveiling the creature to the graveyard no longer exiles it while normal disturb behavior remains intact.
