Description
I noted this today, running 1.21.2. I have and always have had Require approval checkboxes checked; however, my most recent requests have simply been submitted without requiring approval.
I double-checked that I had the latest version with a git pull, and it says I am already up to date, the UI shows v. 1.21.2.
Meteor --version shows 1.2.1

Research direction
Reproduce the issue in version 1.21.2 with the Require approval setting enabled, then trace the request-submission and approval workflow from the UI entry point. Done means new requests remain pending and require approval instead of being automatically approved.
