Description
I can add cookie excludes from the network admin side. The excludes are not added to .htaccess until I go into the main site and save settings there. This is a multi-site install using directories, not subdomains.

Research direction
The issue concerns cookie excludes configured in the WordPress network admin and their absence from .htaccess on directory-based multisite installs. First reproduce the behavior by saving excludes only from network admin, then trace the settings-to-.htaccess update path; done when those excludes are written without saving settings from the main site.
