Add / update docs about exposing web UI and grafana dashboards

In linkerd/linkerd2#2039 we're changing the way in which grafana dashboards are served when running the `linkerd dashboard` command. This has implications for users who want to expose the web/grafana frontends via ingress. We should make sure the documentation in this repo covers it, and we should update the linkerd2 katacoda scenarios as well.
