Private names from `import all`-ed modules are shown as inaccessible

Reproduce the issue using the minimal A.lean and B.lean files, especially the #guard_msgs theorem a_eq in B.lean. Compare the traced goal with the expected output and investigate the name display behavior; done when the goal is shown as a = 5 rather than A.a✝ = 5.
