Description

Problem

Approval request has no timeout. If approval handler never responds, transaction hangs indefinitely.

Recommendation

  • Add configurable timeout to approval flow
  • Throw ApprovalTimeoutError when exceeded
  • Default to reasonable timeout (e.g., 5 minutes)
