Error provisioning

The app keeps popping up (Mac OS) and asks me to login. I do this successfully, but then get an error something like:

Unknown Error: Account already provisioned.

Minor comments:
1). make reporting errors easier (provide link in app, under the error).
2). make reporting bugs (on web page) more prominent (or in welcome email), and actually point to the github issues page :).

Start by reproducing the Mac OS login and provisioning flow described in the issue, recording the exact error and the point at which it appears. Trace the provisioning entry point to determine whether the existing-account case is handled, then verify the corrected flow and consider the requested error and bug-report links separately.
