Metadata Attached to each submitted issue

We should add the following attributes (enabled by default and disabled in settings page) to each issue as as comment:

As a far out feature (probably need to make another issue for this) a right click and quote to issue menu option would be awesome.

Begin by locating the issue-submission flow and the settings page in the Chrome extension. Add the submitting site's metadata as an issue comment, enabled by default but configurable in settings; the proposed right-click quote option is described as a separate future issue.
