Switch to issue forms for issue templates

### Problem

According to the GitHub docs the issue form should now be available in beta: https://docs.github.com/en/communities/using-templates-to-encourage-useful-issues-and-pull-requests/configuring-issue-templates-for-your-repository#creating-issue-forms


### Proposed Solution

We could update the issue templates in this repo to use the form instead. This could make it easier for folks to report issues and submit feature requests.

Probably this was discussed somewhere so if anyone has a link to a relevant discussion feel free to share!
