No save icon, no 'file' menu in notebook

Hmm, I don't seem to have a disc icon in the upper left icon bar? What am I doing wrong? It used to work. Also I can't name my notebook... the code in the notebook works as expected

Also, no 'file' menu item ...?

and, having returned from the holidays I see exactly the same in my work environment on a windows 2019 vm.
