Create array or object based on selection in above div fields

Hi team, As i am using jsoneditor newly I need help from you. I am looking for creating new Array/Object in a jsoneditor based on selection in different div fields. Any suggestion links if someone has done this before would be very helpful.
In detail :
In one div id , i have this selection.

If i select yes here, it should go and add an activity in my jsoneditor with name whatever I want to give like for example: Jira

Finally number of activity in my editor should be 5 with newly added "jira"

Hope I am clear on my question. Anyone here has any idea on it?

Note: I tried OnCreatemenu option, i didn't succeed. May be I am not getting clear picture on how to use in my context.
