Add explanation of knex/ctx/em relationships

"It seems like a lot relies on having knex but the instructions don't make it clear that I need the knex instance every time I also need an entity manager. I suggest adding a simple `knex` accessor to the entity manager so I can pass in the one object and get what I need all in one place."
