Transfer out: django-polymorphic

I would like to move [django-polymorphic](https://github.com/jazzband/django-polymorphic) to the [Django Commons organization](https://github.com/django-commons).

@jezdez I have the ability to transfer ownership across github, pypi and rtd so **if you want to signal approval here I can go ahead and coordinate everything to get this one off your plate**.

### Motivation

With Jazzband sunsetting (https://github.com/jazzband/help/issues/425), django-polymorphic needs a new home. I am currently an admin at django-commons and I maintain two other packages there so this is a natural fit.

### Maintainer agreement

@bckohan — I am the only Jazzband lead so I believe my approval is all we need. However, if other active users (@JohananOppongAmoateng @akx @WhyNotHugo @chrisglass) want to chime in with concurrence here please do so!
