cycle.js app

I was wondering how you could visiualize a complete cycle.js app. I made a simple hello-world testcase:

but it does not seem to process further events.

Note: I get errors with the rx.all.js version in the `vendor` so I used a cdn version (4.0.6).

Start with the hello-world testcase in commit a6f5374a8a3b4dcedb0eff32630b834b4bbc28db and compare its behavior with the rx.all.js file in vendor. Reproduce the failure using the referenced Cycle.js app and the rx 4.0.6 CDN version; done means the app processes events beyond the initial state without the reported errors.
