Column label for tbl_hierarchical_rate_and_count(by=NULL) as 'All Participants'?

Description
When we create a table with `tbl_hierarchical_rate_and_count` with by set as NULL, currently there is no label associated with this, should we set this as "All Participants"? It could be difficult as we might have to allow for users to change this label as per their needs i.e. to "All Subjects", "All Patients", "Total"
