Create empty data dictionary spreadsheets (for LMS and H)

Create a script which can dump out a spreadsheet suitable for creating a data dictionary which contains information about how our data can be handled and what it's for.

Ideally this would allow for maintenance in future when new fields are added and removed.

A nice feature would be if you could run it on an existing spreadsheet and new rows would be added as required without disturbing existing rows (unless they have disappeared).
