Description
Sometimes I want to connect to a proxy that isn't in the hardcoded [list](https://github.com/humanmade/hm-proxy/blob/master/hmproxy#L390-L409), it would be useful if I could pass a fully-qualified hostname to `-r`.
e.g.
```
sudo hmproxy -u tom -r eu.proxy.hmn.md -p 24
```
