Description
[We don't bootstrap Mercator on `upgrade.php`](https://github.com/humanmade/Mercator/blob/master/mercator.php#L47-L50).
If you visit `upgrade.php` on a mapped site, it will fatal thinking multisite is not installed because the mappings aren't considered, and hit `dead_db()`.
