Bug: Change to document message in chat shows incorrectly & with wrong date

**Summary**

When you change a document, a message appears in the chat under “Documents” but it only shows a string of numbers; also, the date is wrong (in this example the change was made on 7/5 but shows as 7/3 in chat)

**Steps to reproduce:**

1. Make a change to a document
2. Check the message in the documents section of chat
3. Click “Show more” to reveal

I tried to recreate the issue on front, but when I change a document in front it doesn’t appear in chat at all. Some older documents with changes do appear with the same issue in the message.
