Description
### Nomad version
0.11.1
### Operating system and Environment details
Photon OS3
### Issue
We deployed a Zookeeper upgrade that failed on a 3 node cluster. The first node failed and Nomad was unable to place the allocation. Nomad did not rollback the node, and the UI reports 3 Healthy allocations (but only 2 are running)

Research direction
Start by reproducing the failed three-node Zookeeper upgrade on Nomad 0.11.1 and trace allocation placement, rollback, and the UI's healthy-allocation count. Done means the failed allocation is no longer reported as healthy and the node and cluster state are consistent after placement failure.
