HackForLA Wiki: New Wiki article on creating issues

### Overview

As a member of this team, I want to make sure that the issue I create follows the guidelines of this project.

### Action Items
- [ ] Create a new Wiki with an appropriate title.
- [ ] Include guidelines on creating issues.
- [ ] Guidelines must include the following information:
- [ ] Every issue needs, **at minimum**, 3 labels: a Feature/P-Feature label, a Size label, and a Role label. More labels beyond that are allowed as appropriate.
