Request full features for ArcGIS survey account

### Overview
We need to request full featured access for ArcGIS survey accounts from Empowerla.org so projects can use it.

### Action Items
- [ ] Collect the following information
- [ ] First Name
- [ ] Last Name
- [ ] Email Address
- [ ] User Name
- [ ] Email the list to Julien from Empower LA and he will create the accounts.
